My daughter(s) has my permission to participate in the 2009 Fast Club Softball Program and I specifically agree to hold harmless the Oshkosh Board of Education, the City of Oshkosh, and all officers, agents, coaches, and members of the same from any and all liability, injury of loss arising out of or occurring in the course of participation in the Fast Club Program.  I certify that this participant is able to participate in this athletic activity.
